Peptides for muscle growth: what the evidence shows

A two-year trial in healthy adults over 60 did exactly what a muscle peptide promises. Lean mass went up. Strength did not. That gap is the most important thing to understand about every peptide on this page.

The answer, in short

Most peptides sold for muscle growth raise the body’s own growth hormone, including sermorelin, CJC-1295, ipamorelin and the oral drug MK-677. In older adults, MK-677 added about a kilogram of lean mass over a year without improving strength or function. IGF-1 variants, MGF and follistatin build muscle in animals but have no human trials as injections.

Why muscle gets harder to build

Ask anyone over 45 who trains seriously and you will hear the same complaint. The work is the same, the protein is the same, and the returns are smaller. Recovery takes a day longer. Muscle that once came in a season now takes a year.

Part of the explanation sits in the hormonal system that peptides target. Growth hormone is released in pulses from the pituitary gland, and it tells the liver and the muscles to make IGF-1, insulin-like growth factor 1, which drives tissue growth. Both fall with age. In a 1992 study, ten healthy older men had lower growth hormone and IGF-1 than young men. When they were given GHRH(1-29), the shortest active piece of the brain’s growth hormone releasing signal, their pituitaries responded like young ones, and two weeks of treatment brought both hormones back to young levels.

Muscle has its own local signals too. After heavy resistance exercise, muscle switches on a stretch-sensitive form of the IGF-1 gene, later named mechano growth factor. In one biopsy study, it rose after heavy exercise in young adults and not in older ones. And every muscle carries a brake, myostatin, that limits how big it can grow. Mice bred without the myostatin gene had individual muscles two to three times normal weight.

So there are three places to push: the growth hormone and IGF-1 axis, the local growth signals inside muscle, and the myostatin brake. The peptides sold for muscle growth each pick one.123

Four families, one goal

The largest family releases your own growth hormone. GHRH analogs, including sermorelin, CJC-1295 and tesamorelin, copy the brain’s releasing signal. Ghrelin mimetics, including GHRP-2, GHRP-6, hexarelin, ipamorelin and the oral small molecule MK-677, act on a second pituitary receptor, the ghrelin receptor GHS-R1a. The two routes add together, which is why the pairs sold online usually combine one of each. Because both work through the pituitary, the body’s own feedback still caps the response.

The second family skips the pituitary and supplies the growth signal directly. IGF-1 LR3 and IGF-1 DES are altered forms of IGF-1 that escape the binding proteins that normally hold IGF-1 in check, so more of it reaches cells. That makes them more potent, and it also removes a safety catch.

The third and fourth families act inside muscle. Mechano growth factor is the stretch-switched piece of the IGF-1 gene product, sold as a synthetic peptide and a pegylated form called PEG-MGF. Follistatin-344 binds myostatin and activin, releasing the brake. Both are early research, and the human data are thin or absent.471211273035

Sermorelin, CJC-1295 and tesamorelin: asking the pituitary

Sermorelin is the oldest of the group. It was once an approved medicine, used to test pituitary function and to help children with growth hormone deficiency grow, and in those children height velocity rose and held through 12 months. In older adults the trials are small. Eleven men aged 64 to 76 took it nightly for six weeks, and nighttime growth hormone rose and some strength measures improved, while IGF-1 and body composition did not change. In a five-month study of a close analog, lean mass and insulin sensitivity rose in men only, and the IGF-1 rise faded over time.

CJC-1295 was built to last. It binds albumin in the blood and has a half-life of about six to eight days. In two placebo-controlled trials in healthy adults, one injection raised growth hormone 2 to 10-fold for six days or more and IGF-1 1.5 to 3-fold for up to 11 days. A second study found the natural pulses persisted, with the baseline between them lifted. No trial has measured muscle or strength.

Tesamorelin is the only one with a current FDA approval, for one narrow use. In a 26-week trial of 412 adults with HIV and excess abdominal fat, visceral fat fell 15.2 percent while it rose 5.0 percent on placebo. Its label lists joint pain, swelling and muscle pain as common side effects and warns about glucose intolerance and fluid retention, which are worth reading as the honest cost sheet for raising growth hormone in general.45678941

MK-677, ipamorelin and the GHRPs: borrowing the hunger signal

The ghrelin mimetics were discovered backwards. GHRP-6 was described in 1984 as a peptide that released growth hormone through an unknown receptor. The receptor was cloned in 1996, and in 1999 researchers found the natural hormone that used it, ghrelin, made in the stomach. That history explains the main side effect of the class. Ghrelin is a hunger hormone, and in seven lean men a GHRP-2 infusion raised food intake at a buffet by about 36 percent.

MK-677 is the best-tested member, and its two-year trial is the most useful study in this chapter. Sixty-five healthy adults aged 60 to 81 took it or placebo. Fat-free mass rose 1.1 kilograms on MK-677 and fell 0.5 kilograms on placebo. Strength and function did not change. Fasting glucose rose, insulin sensitivity fell, and mild leg swelling and muscle pain were common. In a later trial in 123 older adults recovering from hip fracture, gait speed improved but most other function did not, and the study was stopped early because of heart failure.

Ipamorelin was designed to be the clean one. In animals it released growth hormone without raising ACTH or cortisol, even at very high doses, and in healthy men it produced a single growth hormone pulse peaking around 40 minutes with a half-life of about two hours. GHRP-2 and hexarelin, by contrast, raise prolactin and cortisol along with growth hormone. Hexarelin also shows the class’s tolerance problem most clearly. In 12 older adults, its growth hormone response was about 45 percent lower by week 16 and recovered after a four-week break.10121314242519201517

IGF-1 variants, MGF and follistatin: skipping the queue

IGF-1 LR3 and IGF-1 DES are the most potent compounds in this chapter and the least tested in people. Both slip past the binding proteins that normally hold most IGF-1 in reserve. DES was about 10 times more active than IGF-1 on muscle cells in a dish. In pigs and monkeys, these variants lowered blood sugar two to three times more than natural IGF-1, and for much longer. The approved, natural-sequence IGF-1 medicine shows what that means in practice. In a registry of 306 young patients, 80 had at least one low blood sugar event.

Mechano growth factor is the most contested idea here. Early cell work found its peptide raised muscle cell growth independently of the IGF-1 receptor. Later work found the effect depended on that receptor, and a replication study at two drug companies found no apparent effect on muscle cells at all. In mice given sustained gene delivery of a related E peptide, muscles grew larger and weaker.

Follistatin has the most dramatic animal data. Mice engineered to make extra follistatin in muscle gained striking mass. In people, it has been tested only as gene therapy, in six adults with Becker muscular dystrophy, where four walked farther, and in a small inclusion body myositis trial whose findings were disputed in print. When anti-doping scientists bought 17 black-market follistatin products, only 9 contained follistatin.262829313233343536373839

Mass on the scan, strength in the hand

Here is the pattern that runs through this whole chapter, and it is worth repeating to anyone who asks which peptide builds muscle. When growth signals are pushed from the outside, the scan moves more reliably than the barbell. MK-677 raised fat-free mass for two years without improving strength or function. An E peptide grew mouse muscle while making it weaker. Follistatin grew mouse muscle while weakening bone. Even in an eight-week MK-677 study in obese men, fat-free mass rose while glucose tolerance worsened.

Part of the reason is what a body composition scan counts. Fat-free mass is everything that is not fat, including water, and growth hormone holds salt and water. The leg swelling in the MK-677 trial and the fluid retention warning on the tesamorelin label are two views of the same effect. Some of the lean mass these compounds add is real tissue, some is fluid, and the scan cannot tell them apart.

The deeper reason is that strength is a property of trained tissue. Nerves learn to recruit fibers, tendons stiffen, and the fibers themselves remodel in response to load. A hormone can make the building materials more available, and only training tells the body what to build with them. The one older-adult sermorelin study that found some strength gains was also the one where body composition did not change, which is a useful reminder that the two measures can move separately.24344023415

What else moves the same system, and what to measure

The largest natural growth hormone pulse comes in early deep sleep. MK-677 increased the deepest stage of sleep by about half in young adults, while hexarelin given in the evening reduced it and raised cortisol. Before reaching for a growth hormone releaser, it is worth asking how well you sleep, because sleep is the body’s own releaser.

Resistance training is the other lever every study in this chapter assumes. It is what switches on the muscle’s local growth signals, and it is the only thing in this chapter shown to turn growth signals into strength. Protein supplies the material. In one short MK-677 study, dieting volunteers went from losing protein to a slight positive balance, which shows how closely this axis is tied to nutrition.

The labs follow the side effects. Growth hormone and its mimics raise blood sugar and lower insulin sensitivity, so HbA1c and fasting glucose belong on the baseline. GHRP-2 and hexarelin raise prolactin and cortisol. Testosterone is the other major anabolic hormone, and a low level is a simpler explanation for stalled progress than a missing peptide.21182222415

Where these stand in 2026

Tesamorelin is FDA-approved for one use, reducing excess abdominal fat in adults with HIV-associated lipodystrophy, and its label says it is not for weight loss. Sermorelin was approved and later discontinued by its maker. None of the others is approved, and none is among the six peptides covered by FDA’s October 2026 enforcement discretion for compounding. FDA’s compounding safety page flags CJC-1295, ipamorelin, MK-677 and PEG-MGF over safety concerns. All of them are prohibited in tested sport.4142444345

What this means for you

The growth hormone releasers reliably raise growth hormone and IGF-1, and in older adults they add some lean mass. What they have not shown in controlled trials is the thing most people want, more strength and better function. They also carry a predictable cost in blood sugar, fluid and, for some, cortisol and prolactin. The direct growth signals are more potent and much less tested, with real concerns about blood sugar and product quality.

The best question to ask a clinician is what is actually limiting your progress. Low testosterone, poor sleep, too little protein and an unprogressive program are all common, measurable and fixable. If a growth hormone releaser still makes sense after that, a baseline of glucose, HbA1c and hormones lets you see what it changed beyond the mirror.242839

Questions

What is the best peptide for muscle growth?

No peptide has shown strength gains in a large controlled trial. MK-677 added about a kilogram of lean mass over a year in older adults without improving strength. Sermorelin and CJC-1295 raise growth hormone in small studies. A clinician can weigh the options against your labs.

Do growth hormone peptides build muscle?

They raise growth hormone and IGF-1. In older adults, that has added some lean mass, which includes water, but trials have not shown better strength or function.

Why are CJC-1295 and ipamorelin taken together?

They act on two different pituitary receptors, the GHRH receptor and the ghrelin receptor, and the two routes add together. No published trial has tested the combination for muscle.

Is MK-677 a peptide?

No. MK-677 (ibutamoren) is an oral small molecule that mimics ghrelin. Trials found more lean mass, more appetite, higher fasting glucose and, in a hip fracture trial, a heart failure signal.

Does IGF-1 LR3 build muscle in humans?

It has never been tested in people. In animals it is more potent than natural IGF-1 and lowers blood sugar more strongly and for longer.

Does follistatin increase muscle in people?

It has been tested only as gene therapy in small muscle disease trials. In a lab study of 17 black-market follistatin products, only 9 contained follistatin.

Do growth hormone peptides stop working over time?

Some do. With hexarelin, the growth hormone response was about 45 percent lower by week 16 and recovered after a four-week break. Sermorelin-type IGF-1 rises also faded over months.

Are muscle peptides allowed in sport?

No. Growth hormone releasers, IGF-1 variants, MGF and follistatin are all on the World Anti-Doping Agency’s Prohibited List.
