# Medical Partners

## Medical and service partners

Vitals Vault provides technology, health-data tools, non-clinical administrative support, and service coordination. Vitals Vault does not practice medicine, employ the clinicians described below, make prescribing decisions, or dispense medication.

## Independent medical groups

- **AYARX Providers PC** - California professional corporation and independent medical group whose licensed clinicians may review an intake and make clinical decisions. The medical group responsible for a particular service is identified before the member begins intake and again before consent and payment. The member's clinician-patient relationship is with the assigned clinician and medical group, not with Vitals Vault. Each clinician must be licensed in the state where the member is physically located at the time of care.

## How prescription charges are separated

The medical consultation fee is a separate charge for services provided by the assigned medical group and is collected by Vitals Vault on that medical group's behalf. If a clinician prescribes treatment, the pharmacy or medication plan price is charged separately after approval. The medical consultation fee is not a deposit or credit against the medication charge. If the clinician does not prescribe treatment, the medical consultation fee is refunded as shown during checkout.

## Dispensing pharmacy

Each partner is responsible for its own professional services, licensure, notices, and legal obligations. Partner availability varies by service and member location.
