# Checkout and Intake Terms | Vitals Vault

# Checkout and Intake Terms

The charges, clinical review, renewals, cancellations, and refunds that apply when you request prescription treatment.

## The short version

- The medical consultation fee and medication plan price are two separate charges.
- The consultation fee pays for independent clinical review and is collected at intake on behalf of the assigned medical group.
- The medication plan price is not part of the consultation charge. It is charged separately if a clinician prescribes treatment.
- If the clinician does not prescribe treatment, the consultation fee is refunded to the original payment method.

## 1. Clinical intake and eligibility

Medication checkout is currently for adults 18 or older requesting treatment for themselves. You must provide complete and accurate identity, location, medical history, medication, allergy, and other health information requested during intake. An independent licensed clinician reviews that information and decides whether to prescribe. The clinician may ask for more information, recommend another option, or decline treatment. Completing checkout does not guarantee a prescription.

## 2. Two separate charges

The amount identified during checkout as the medical consultation fee is charged at intake. Vitals Vault collects it on behalf of the medical group assigned to your service. It pays for clinical review and is not a deposit, coupon, or credit against medication.

If a clinician prescribes treatment, the full medication plan price shown at checkout is charged separately before pharmacy fulfillment. The consultation fee is not deducted from that medication charge. If a clinician does not prescribe treatment, the consultation fee is refunded to the original payment method. Your bank may take additional time to post the refund.

## 3. Independent partners

The assigned clinician and medical group make all clinical decisions. MDI Medical Group, PC or AYARX Providers PC may be assigned, as identified before intake, consent, and payment. Strive Specialties, Inc., doing business as Strive Pharmacy, dispenses prescribed medication when authorized to serve your location. Vitals Vault provides technology, administrative support, and payment coordination; it does not practice medicine or dispense medication. ## 4. Renewals and cancellation

If you choose a renewing medication plan, checkout identifies the full medication plan price and renewal schedule. We may charge the payment method on file at each renewal until you cancel. Cancellation stops future renewals but does not undo a consultation already provided or medication already ordered, dispensed, shipped, or otherwise made available. Follow the cancellation method shown in your account or contact support@vitalsvault.com.

## 5. Medication issues and refunds

Prescription medication generally cannot be returned or restocked after it has been dispensed. Contact support promptly if medication is damaged, incorrect, missing, adulterated, recalled, or appears to have been unlawfully supplied. We will review the issue with the dispensing pharmacy and provide the remedy required by applicable law and the terms shown for your purchase.

## 6. Other terms

The product, price, cadence, consent, and disclosures displayed in your exact checkout are also part of the terms for that purchase.
