# Peptides for gut health

Your gut lining is one cell thick and talks to the rest of your body in peptides. That is why a molecule borrowed from it can calm it, and why more is rarely better.

## The answer, in short

The peptides most discussed for gut health are KPV, which turns down inflammation inside gut and immune cells, and BPC-157, which protects the gut lining and its blood vessels in animal studies. Neither has published controlled human trials. The gut peptides with strong human evidence, teduglutide and linaclotide, copy hormones the gut already makes and are prescription medicines for specific diagnoses.

## A gut that will not settle is a systems problem

The story usually goes like this. A run of antibiotics, a hard training block, a month of ibuprofen for a sore knee, and afterward the gut never quite goes back to normal. Bloating after meals. Loose stools one week and stubborn ones the next. A growing list of foods that seem to cause trouble. Somewhere in the search for answers, the word peptide shows up.

To see why, it helps to know what is being protected. The intestinal barrier has three layers, a coat of surface mucus, a single sheet of epithelial cells, and the immune defenses packed beneath them. The cells in that sheet are stitched together by tight junctions, protein seals that decide what slips between them. Zonulin, the only physiological regulator of those seals described in a 2011 review, can open them, and when that pathway goes wrong in susceptible people the result is linked to inflammation and autoimmunity inside and outside the gut.

Plenty of ordinary stresses loosen the barrier. A 2019 Mayo Clinic review for clinicians listed endurance exercise, NSAID painkillers, pregnancy, bile acids and food emulsifiers among the things that raise intestinal permeability. The same review drew a hard line that is worth keeping in mind for the rest of this chapter. Inflammatory and ulcerating diseases make the gut leaky, but no such disease has been shown to clear up by simply restoring the barrier.

So gut health is less a single target than a conversation between lining, blood supply, immune cells and microbes. The peptides worth knowing each speak to one part of that conversation.[1](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-camilleri-2019)[2](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-fasano-2011)

## The gut already runs on peptides

Long before anyone sold a peptide vial, the gut was making its own. Scattered through the lining are enteroendocrine cells, chemical sensors that taste what you eat and answer with a whole family of hormones. A 2016 Cambridge review describes how those hormones fine-tune responses to food inside and outside the gut, from digestion to blood sugar to appetite. GLP-1, the hormone behind the newest weight-loss medicines, is one of them.

Its sibling, GLP-2, was found almost by accident. In 1996, a Toronto team noticed that mice carrying tumors that overproduced the glucagon gene had remarkably overgrown small intestines. The culprit was GLP-2, a peptide with no known job until then. Given to mice, it made the crypts of the intestine divide and the villi grow, and the bowel gained weight within four days.

Peptides that act on the gut tend to work through one of four routes.[3](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-gribble-2016)[4](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-drucker-1996)

- 01

### Grow the lining

GLP-2 and its long-acting copy, teduglutide, drive growth of the absorbing surface of the small intestine.[4](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-drucker-1996)[5](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-jeppesen-2012)

- 02

### Seal the junctions

Larazotide was designed to regulate tight junctions, the seals between lining cells that zonulin can open.[8](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-leffler-2015)[2](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-fasano-2011)

- 03

### Quiet the alarm

KPV enters gut cells and turns down NF-κB, a master switch for inflammatory genes.[10](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-dalmasso-2008)

- 04

### Protect the supply lines

BPC-157 protects the small blood vessels of the stomach and gut lining in animal models.[13](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-sikiric-2006)

## KPV, the peptide that inflammation lets in

KPV is three amino acids, lysine, proline and valine, clipped from the tail of alpha-MSH, a hormone best known for tanning skin that also calms immune cells. Its size is its trick. Gut cells absorb small peptides from digested food through a transporter called PepT1, and in 2008 an Atlanta team showed that PepT1 carries KPV inside, where it blocks NF-κB and MAP kinase, two of the main inflammation switches.

PepT1 is scarce in a healthy colon and rises when the colon is inflamed. A molecule that enters through that door is absorbed more where inflammation is and less where it is not. In mice, KPV in drinking water reduced chemically induced colitis, and a German group found the same in a second model, with faster recovery, weight regain and fewer inflammatory cells in the colon wall.

The limits are just as clear. Every colitis result comes from mice. A 2016 mouse study found that high PepT1 promoted colitis-linked tumors, while KPV itself prevented them, and the effect vanished in mice without the transporter. No controlled human trial of KPV has been published.[10](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-dalmasso-2008)[11](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-kannengiesser-2008)[12](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-viennois-2016)

## BPC-157, back where it started

BPC-157 is better known today as a tendon peptide, but it began in the stomach. It is a 15-amino-acid fragment of a protective protein in human gastric juice, unusually stable in acid, and its first tests were in the gut. Given into the stomachs of rats exposed to alcohol, it kept the small blood vessels of the stomach intact and held back lesions of the stomach, esophagus and duodenum.

One line of animal work speaks to a very common story. NSAIDs, the anti-inflammatory painkillers, injure the gut lining over time, and in experimental models BPC-157 counteracted that damage. In other words, the peptide studied for tendon pain blunted the side effects of the pills people take for tendon pain, at least in rats.

The gut is also where BPC-157 went furthest in people. Under the code PL 14736 it entered inflammatory bowel disease trials in Croatia, which its developers describe as safe. Full trial results have not been published, which is why the evidence label still reads early research.[13](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-sikiric-2006)[14](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-sikiric-2013)[15](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-vasireddi-2025)

## The gut peptides that became medicines

If you want to know what a gut peptide looks like when it finishes the journey, look at the two that did. Both copy signals the gut already sends.

Teduglutide is a long-acting version of GLP-2, the hormone that made those mouse intestines grow. In a 24-week randomized trial in people with short bowel syndrome who depended on intravenous nutrition, 63% of those on teduglutide cut their IV support by more than a fifth, compared with 30% on placebo. Citrulline, a blood marker of how much absorbing lining a person has, went up.

Linaclotide acts on guanylate cyclase-C, the receptor that the gut’s own hormones guanylin and uroguanylin use to control fluid, the barrier and repair of the lining. In an 804-person phase 3 trial in irritable bowel syndrome with constipation, 33.7% on linaclotide met the strict combined pain and bowel goal, against 13.9% on placebo, with diarrhea the main side effect.

Larazotide, the tight junction peptide, shows how hard the last mile is. In a 342-person trial in celiac disease, it reduced symptoms in people already on a gluten-free diet, but only at one dose, and the trial’s own authors called the results mixed. It has not become a medicine.[5](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-jeppesen-2012)[4](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-drucker-1996)[7](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-waldman-2018)[6](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-chey-2012)[8](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-leffler-2015)

## Why more is rarely better with gut peptides

Look closely at the human trials and a pattern appears that runs against how most people think about supplements. In the larazotide trial, three doses were tested. The lowest one beat placebo, with fewer symptom days, less abdominal pain and even less headache and tiredness. The two higher doses did no better than placebo on any measure.

LL-37, an antimicrobial peptide your white blood cells and skin release, showed a similar shape. In its first human trial, a gel on hard-to-heal leg ulcers, the two lower strengths sped healing several-fold over placebo. The highest strength showed no difference, and a larger follow-up trial missed its overall goal.

Nobody knows for certain why. A likely explanation is that these peptides are signals, and the gut is a finely balanced system of seals, immune cells and microbes that reads signals in context. Push a signal too hard and the system may tune it out or answer with something else. Whatever the reason, it is the best argument in this chapter against the instinct to take more of a gut peptide when a little seems to help. The only peptides proven in the gut were tested dose by dose in people, and the right answer was not always the biggest one.[8](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-leffler-2015)[16](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-gronberg-2014)[17](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-mahlapuu-2021)

## What else moves the gut, and the labs to check

Some of the strongest levers on the gut lining are already on your plate. Fiber is fermented by gut bacteria into butyrate, the main fuel of colon cells. A 2008 review found that butyrate calms inflammation and reinforces the colon’s barrier, and one of its main mechanisms is blocking NF-κB, the same inflammation switch KPV targets. Human data are still limited, but a wide range of plant fibers is a cheap way to work on the same pathway.

Your own gut peptides need inputs too. Vitamin D directly switches on the gene for LL-37 in immune cells, so a low vitamin D level means less of an antimicrobial peptide your body already relies on. And because NSAIDs, endurance exercise and food emulsifiers all raise permeability, the first question for a gut that will not settle is what keeps loosening it.

Labs give the conversation numbers. hs-CRP shows whether inflammation is spilling into the whole body. Ferritin and albumin reveal whether an irritated gut is absorbing iron and protein. Vitamin D tells you whether the LL-37 switch has what it needs. A baseline and a retest are the honest way to judge any gut peptide.[9](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-hamer-2008)[18](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-gombart-2005)[1](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-camilleri-2019)[10](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-dalmasso-2008)

## Where gut peptides stand in 2026

On October 29, 2026, NCPA reported that FDA granted enforcement discretion allowing 503A compounding pharmacies to prepare KPV and BPC-157, along with four other peptides, for individual patients with a prescription. Neither is FDA-approved for any condition, and final rulemaking is pending. FDA’s April 2026 safety page had noted how little human exposure data exists for these substances. LL-37 is not covered, and research-use vials are not made for people. Teduglutide and linaclotide are approved prescription medicines for the specific diagnoses they were tested in.[19](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-ncpa)[20](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-fda-bulks)[21](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-fda-503a)

## What this means for you

The gut talks in peptides, and the peptides that have proven themselves in people are copies of its own hormones, tested for specific diagnoses. KPV and BPC-157 are interesting for good biological reasons, a transporter that inflammation opens and a lining whose blood supply they protect, but their case still rests on animals.

If you have a diagnosed inflammatory bowel disease, any peptide belongs alongside your current care, never in place of it. For everyone else, the question to bring to a clinician is simple and specific. What is loosening my gut lining, and which of my labs would show that a peptide changed anything?[11](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-kannengiesser-2008)[13](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-sikiric-2006)[1](https://www.vitalsvault.com/peptides/peptides-for-gut-health#source-camilleri-2019)

## Questions

What is the best peptide for gut health?

No peptide has proven itself for general gut health in a controlled human trial. KPV and BPC-157 are the most discussed, both with mainly animal evidence. The gut peptides with strong human data, teduglutide and linaclotide, are prescription medicines for short bowel syndrome and constipation-type IBS.

Is KPV or BPC-157 better for the gut?

They work differently. KPV turns down inflammation inside gut and immune cells, and BPC-157 protects the lining and its small blood vessels in animal studies. No study has compared them. Whether your problem looks more like ongoing inflammation or damage to the lining helps a clinician choose.

Can peptides heal leaky gut?

Larazotide, a peptide designed to regulate the seals between gut cells, reduced celiac symptoms at its lowest dose in one trial. Beyond that, no peptide has been shown to repair a leaky gut in people, and a Mayo Clinic review notes that restoring the barrier alone has not been shown to resolve disease.

Do gut peptides work by mouth?

Some do. KPV is absorbed by a gut transporter called PepT1 and reduced colitis in mice when given in drinking water. BPC-157 is unusually stable in stomach acid. Linaclotide is an oral peptide that acts on the gut lining without needing to be absorbed.

Is LL-37 good for gut health?

LL-37 is an antimicrobial peptide your white blood cells and skin release, and vitamin D switches on its gene. Antimicrobial peptides as a group are concentrated in the gut’s mucus layer. As a product, it has only been tested as a gel on leg ulcers. Oral and injected LL-37 have not been tested in people.

What labs should I check for gut health?

hs-CRP shows whether inflammation is affecting the whole body. Ferritin and albumin show whether you are absorbing iron and protein well. Vitamin D matters because it switches on one of your own antimicrobial peptides. Retesting the same markers shows whether anything changed.

Are gut peptides safe with inflammatory bowel disease?

KPV and BPC-157 have not been tested in controlled human IBD trials. If you have Crohn’s disease or ulcerative colitis, keep your prescribed treatment and discuss any peptide with the clinician who manages your bowel disease.
