## MOTS-c, in short

MOTS-c is a 16-amino-acid peptide coded in mitochondrial DNA and discovered in 2015. It acts mainly on skeletal muscle, switching on AMPK, the cell’s energy sensor, and improving how muscle uses glucose. Exercise raises its levels in people. It is studied for insulin sensitivity, weight and physical decline with age, so far mostly in mice.

## How does MOTS-c work?

MOTS-c acts mainly on muscle, and its effects overlap with what exercise does.

-

### Flips on the energy sensor

It switches on the same energy sensor in cells that exercise turns on.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)

-

### Helps muscle use sugar

In mice, it helped the body handle sugar and prevented insulin resistance, mostly through muscle.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)

-

### Sends stress signals to the core

When cells are under stress, it moves into the cell’s control center and helps turn on protective genes.[4](https://www.vitalsvault.com/peptides/mots-c#source-kim-2018)

## MOTS-c benefits: what it is studied for

The strongest results come from mice. Human research so far measures natural MOTS-c levels and genes, not treatment.

## A hormone hiding inside your mitochondria

Nearly every one of your cells carries two genomes. The big one sits in the nucleus. The small one, a little circle of DNA, sits inside each mitochondrion, the structures that turn food and oxygen into usable energy. For decades, that small genome was thought to code only for parts of the energy machinery and the RNA needed to build them.

Then a peptide called humanin, discovered in 2001, turned out to be coded inside mitochondrial DNA. If one hidden peptide existed, a team at the University of Southern California reasoned, there might be others. In 2015, led by Changhan Lee and Pinchas Cohen, they found one tucked inside the gene for 12S ribosomal RNA.

They named it MOTS-c, for mitochondrial open reading frame of the 12S rRNA-c. It is 16 amino acids long. In mice it acted on skeletal muscle and prevented both the insulin resistance that comes with age and the insulin resistance and weight gain that come with a high-fat diet.

The finding reframed what mitochondria are. They make energy, and they also send messages to the rest of the body using peptides coded in their own DNA. By 2022, eight such mitochondrial-derived peptides had been published, and MOTS-c had become the one most closely tied to muscle and metabolism.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[9](https://www.vitalsvault.com/peptides/mots-c#source-miller-2022)

## How MOTS-c works: the folate cycle, AMPK and the nucleus

In cell experiments, MOTS-c slows the folate cycle and the purine building that depends on it. Purines are the chemical building blocks of DNA, RNA and the cell’s energy currency. That shift in the cell’s chemistry leads to the activation of AMPK.

AMPK is the cell’s fuel gauge. When energy runs low, as it does during exercise, AMPK switches on and tells the cell to take up more glucose, burn more fat and make fewer energy-expensive products. In cells and mice, MOTS-c flips that same switch without the workout, a likely reason it improved glucose handling. Whether it does the same when given to people is untested.

In 2018, the same USC group found a second act. Under metabolic stress, such as glucose restriction, MOTS-c travels from the cytoplasm into the cell nucleus, in an AMPK-dependent way. There it regulates a broad set of nuclear genes, including antioxidant response genes, and interacts with NRF2, a master regulator of cellular defense.

That second finding was a first of its kind. Scientists had long known the nucleus controls the mitochondria. MOTS-c was the first factor shown to be coded in mitochondrial DNA and to actively control genes in the nucleus. The two genomes, the authors concluded, talk in both directions.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[4](https://www.vitalsvault.com/peptides/mots-c#source-kim-2018)

## MOTS-c for insulin resistance and weight

Skeletal muscle is one of the body’s main sinks for glucose once insulin rises. When muscle stops responding well to insulin, blood sugar and insulin climb, and the path toward type 2 diabetes begins. That makes muscle the logical target for a metabolic peptide, and in the discovery study it appeared to be MOTS-c’s main target.

In the 2015 Cell Metabolism study, MOTS-c treatment in mice prevented insulin resistance that develops with age and insulin resistance driven by a high-fat diet. It also prevented the weight gain that diet normally causes. These are prevention results, which spare the mice a problem rather than reverse one already there.

A 2024 review in the journal Peptides pulled together work on mitochondrial-derived peptides across type 2 diabetes, type 1 diabetes and gestational diabetes. Its authors noted that the amino acid sequences of these peptides have been conserved through evolution, which suggests they do jobs worth keeping.

What is missing is a human trial testing whether MOTS-c, given as a treatment, improves insulin sensitivity or weight in people. The strongest human evidence comes from a different direction, natural variation in the MOTS-c gene itself.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[10](https://www.vitalsvault.com/peptides/mots-c#source-kal-2024)

## MOTS-c and exercise: why it is called an exercise mimetic

In 2021, the USC group and collaborators in New Zealand published a study in Nature Communications that tied MOTS-c directly to exercise. In people, exercise raised MOTS-c levels both in skeletal muscle and in the blood. Your muscles make more of this mitochondrial message when you move.

In mice, MOTS-c treatment improved physical performance at three ages, young at two months, middle-aged at twelve months and old at twenty-two months. It regulated nuclear genes for metabolism and protein quality control, changed muscle metabolism and helped muscle cells adapt to metabolic stress.

The most striking result came from the oldest animals. Mice that started intermittent MOTS-c treatment late in life, at about 23.5 months, which is very old for a mouse, gained physical capacity and healthspan. For a field focused on preventing decline, a benefit started this late is unusual.

This is the root of the exercise mimetic label. MOTS-c switches on some of the same machinery exercise does, and exercise raises it. A more accurate reading is that MOTS-c is part of how exercise talks to the body, which makes it a candidate for amplifying training more than for replacing it.[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)

## One letter of DNA, and why moving matters

Some of the best human evidence for MOTS-c comes from a single change in mitochondrial DNA. In some people of East Asian descent, one letter in the MOTS-c gene is different, a variant called m.1382A>C. It swaps the 14th amino acid of the peptide, making a version called K14Q that has less biological activity.

A 2021 analysis of three cohorts, 27,527 people in Japan and the United States, found that men carrying this variant had a higher prevalence of type 2 diabetes. Women did not. In one Japanese cohort, J-MICC, the extra risk showed up only in men in the least physically active third.

Mouse work matched the human data. Male mice on a high-fat diet given normal MOTS-c weighed less and handled glucose better. Male mice given the K14Q version did not, and female mice were unaffected either way. The authors described this as a kinesio-genomic interaction, a gene whose effect depends on how much you move.

A related study found that carriers of the same variant had more fast-twitch muscle fibers, and the variant was more common in sprint and power athletes than in endurance athletes. Read together, these studies suggest that MOTS-c shapes how muscle handles fuel, and that physical activity can compensate for a weaker version. That is still a hypothesis, but it comes from human genetics.[6](https://www.vitalsvault.com/peptides/mots-c#source-zempo-2021)[7](https://www.vitalsvault.com/peptides/mots-c#source-kumagai-2022)

## MOTS-c and healthy aging

Mitochondrial-derived peptides have drawn steady interest from longevity researchers. A 2022 review in the Journal of Clinical Investigation described how these peptides may connect mitochondrial decline to type 2 diabetes, cancer and neurodegenerative disease.

MOTS-c has also been linked to exceptional longevity. In 2015, Japanese and Spanish researchers suggested that the same East Asian variant might be one of the biological factors behind the long lifespans of Japanese people, while noting that more research is needed. That idea sits in some tension with the diabetes finding, which is a reminder of how early this science is.

What the aging research supports so far is a direction. In mice, MOTS-c improved physical capacity even when started late in life, and in people its production responds to exercise. Whether giving MOTS-c to older adults changes how they age has not been tested.

There is a practical reading of this for anyone over 50. In the human data so far, exercise is what raises your own MOTS-c, and the same exercise builds the muscle that MOTS-c acts on. Any decision about adding the peptide is best made on top of that foundation.[9](https://www.vitalsvault.com/peptides/mots-c#source-miller-2022)[8](https://www.vitalsvault.com/peptides/mots-c#source-fuku-2015)[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)

## MOTS-c vs humanin and the other mitochondrial peptides

MOTS-c has relatives. Humanin and six small humanin-like peptides, called SHLP1 to SHLP6, are coded in the gene for 16S ribosomal RNA. MOTS-c is the only published one coded in the neighboring 12S gene. Together they make up the eight mitochondrial-derived peptides published as of 2022.

Mitochondria read their own genes with a slightly different dictionary, in which some three-letter words that mean stop in the nucleus code for an amino acid, and some that code for an amino acid in the nucleus mean stop. MOTS-c adds a twist: its discoverers reported that it is built outside the mitochondrion using the standard code, even though its gene sits in mitochondrial DNA.

In practice, the family members point in different directions. Humanin was found in a screen for molecules that protect neurons and is studied mostly in brain and metabolic research, largely in animals. MOTS-c is the muscle and exercise peptide of the group, acting directly on how skeletal muscle uses fuel. Both are studied for healthy aging, and neither has published human treatment trials.[10](https://www.vitalsvault.com/peptides/mots-c#source-kal-2024)[9](https://www.vitalsvault.com/peptides/mots-c#source-miller-2022)[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)

## What we know about MOTS-c in people

Human evidence for MOTS-c falls into two groups. One is physiology, showing that exercise raises MOTS-c in muscle and blood. The other is genetics, showing that a less active version of the peptide tracks with type 2 diabetes in inactive men and with muscle fiber type.

Neither is a treatment trial. No peer-reviewed study has yet reported what happens when people are given MOTS-c, how it affects insulin sensitivity or body weight, or how safe it is over months. FDA’s April 2026 compounding page noted the absence of human exposure data.

It is also fair to know who did the work. Much of the MOTS-c research comes from one USC-centered network, and several authors disclose consulting roles and stock in CohBar, the company to which UCLA licensed intellectual property on humanin, MOTS-c and the SHLPs. That does not undo the findings, which appeared in leading journals, but independent replication matters.[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)[6](https://www.vitalsvault.com/peptides/mots-c#source-zempo-2021)[7](https://www.vitalsvault.com/peptides/mots-c#source-kumagai-2022)[12](https://www.vitalsvault.com/peptides/mots-c#source-fda-bulks)[9](https://www.vitalsvault.com/peptides/mots-c#source-miller-2022)

## MOTS-c side effects and safety

Because MOTS-c has no published human safety trials, its side effects in people are unknown. FDA’s April 2026 compounding page made the same point. The mouse study abstracts do not describe harms, but those studies were not designed to look for them.

One theoretical concern follows from the mechanism, though it has not been studied in people. A peptide that improves how muscle clears glucose could add to the effect of insulin, sulfonylureas or other glucose-lowering medicines. Anyone taking them should involve their prescribing clinician and monitor blood sugar.

Quality is the other practical risk. Compounded MOTS-c from a licensed 503A pharmacy is a different product from a vial sold online for research use. People who should talk to a clinician first include anyone on diabetes medicine, anyone pregnant or breastfeeding, anyone with kidney or liver disease, and tested athletes.[12](https://www.vitalsvault.com/peptides/mots-c#source-fda-bulks)[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[10](https://www.vitalsvault.com/peptides/mots-c#source-kal-2024)

## Labs that give context around MOTS-c

MOTS-c is studied for how the body handles glucose, so the useful labs measure that system. HbA1c reflects your average blood sugar over about three months, which makes it useful general context on long-term glucose. No validated protocol uses it to judge whether MOTS-c helps.

Fasting insulin often rises years before fasting glucose does, because the pancreas works harder to keep sugar normal as muscle becomes resistant. Paired with fasting glucose, it can estimate insulin resistance, though those estimates have limits and are not a test for selecting or monitoring MOTS-c. ALT, a liver enzyme, adds general context on liver health, since the liver also handles glucose and fat.

This is how Vitals Vault approaches peptide care. A blood panel at Quest Diagnostics comes first, a licensed clinician reviews your results and history and decides whether MOTS-c fits, and the course ends with a retest of the same markers.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)

## Where MOTS-c stands in 2026

On October 29, 2026, NCPA reported that FDA granted enforcement discretion allowing 503A compounding pharmacies to prepare MOTS-c, along with five other peptides, for individual patients with a prescription. There is no end date, and final rulemaking is pending. MOTS-c is still not an FDA-approved drug.

Athletes face a stricter rule. The World Anti-Doping Agency’s 2026 Prohibited List names MOTS-c as an AMPK activator under metabolic modulators, prohibited in and out of competition.[11](https://www.vitalsvault.com/peptides/mots-c#source-ncpa)[12](https://www.vitalsvault.com/peptides/mots-c#source-fda-bulks)[14](https://www.vitalsvault.com/peptides/mots-c#source-wada)

## What this means for you

MOTS-c is a message from your mitochondria to your muscles, coded in DNA your mother passed down and released when you exercise. In mice it prevents insulin resistance and weight gain and keeps aging animals more capable. In people, genetics and exercise studies suggest it matters, but no published trial has tested it as a treatment.

If you are considering it, the sharper question is how much of what MOTS-c does you are already getting. Regular training raises your own MOTS-c, and the genetic data hint that movement can make up for a weaker version. A clinician can look at your metabolic health and the proven options first, though no test can tell you in advance whether MOTS-c would add anything on top.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)[6](https://www.vitalsvault.com/peptides/mots-c#source-zempo-2021)

## What to expect with MOTS-c

A responsible start includes a baseline HbA1c, fasting insulin and glucose, a review of any diabetes medicine, and a clinician’s decision on whether MOTS-c fits. The prescriber sets the course, and it ends with a retest of the same markers.

On timing, there are no human treatment data. HbA1c reflects about three months of blood sugar, so a retest at that interval is the earliest point a lab change could be judged. Keep training throughout, since exercise raises your own MOTS-c and the genetic data suggest activity shapes how much it matters.[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)[6](https://www.vitalsvault.com/peptides/mots-c#source-zempo-2021)

## MOTS-c myths and evidence

-

### MOTS-c is exercise in a vial

It switches on AMPK, as exercise does, and exercise raises it. But it is one of many signals exercise sends, and human trials of MOTS-c as a treatment have not been published.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)

-

### There is no human evidence for MOTS-c at all

There is human evidence, of a specific kind. Exercise raises MOTS-c in human muscle and blood, and a less active natural variant is linked to type 2 diabetes in inactive men. What is missing is treatment trials.[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)[6](https://www.vitalsvault.com/peptides/mots-c#source-zempo-2021)

-

### Mitochondrial DNA only codes for energy machinery

MOTS-c and humanin are peptides coded in mitochondrial DNA that act like hormones, and MOTS-c can enter the nucleus and regulate nuclear genes.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[4](https://www.vitalsvault.com/peptides/mots-c#source-kim-2018)[9](https://www.vitalsvault.com/peptides/mots-c#source-miller-2022)

-

### MOTS-c is allowed in sport because the body makes it

WADA’s 2026 Prohibited List names MOTS-c as an AMPK activator, banned at all times.[14](https://www.vitalsvault.com/peptides/mots-c#source-wada)

## How is MOTS-c taken?

Compounded MOTS-c is prepared as an injection under the skin. Your prescriber sets the dose and schedule, often alongside exercise and nutrition goals.

## MOTS-c dosage

There is no FDA-approved MOTS-c dose, and no human dose has been published. In its July 2026 review, FDA found no clinical studies giving MOTS-c to people, and online dosage charts are converted from mouse studies. A placebo-controlled trial in adults with prediabetes began in 2026, with results not yet reported. Your clinician sets any dose.[1](https://www.vitalsvault.com/peptides/mots-c#source-fda-pcac-motsc)[2](https://www.vitalsvault.com/peptides/mots-c#source-ct-mots-met)

## Typical MOTS-c dosage

No human dose of MOTS-c has been published. FDA reviewed MOTS-c for compounding in July 2026 and found no clinical studies giving it to people. A company-run, placebo-controlled trial of a once-daily injection under the skin in adults with prediabetes started in 2026, but its dose and results have not been reported. A clinician would have to decide any use and dose.

These are reference doses from FDA labels, studies in people and clinical practice reports, shown for education. They are not a recommendation or a prescription. Your dose, form and schedule should be set by a licensed clinician who knows your history and lab results.

Already have a prescribed dose? The [peptide dosage calculator](https://www.vitalsvault.com/peptides/tools/dosage-calculator) turns it into the units to draw on an insulin syringe.

No dose has been published from studies in people yet, so there is no typical dose to show. A prescribing clinician decides whether MOTS-c fits and how it is given.

## Is MOTS-c right for you?

A clinician makes the call. These are the questions they will ask first.

### It may suit

- Adults working on insulin resistance or metabolic health alongside training
- People who want to track HbA1c and fasting insulin over time
- Healthy-aging goals focused on muscle and energy

### Talk to a clinician first if you are

- Taking insulin or other glucose-lowering medicine
- Tested athletes (WADA prohibits it)
- Pregnant or breastfeeding
- Living with kidney or liver disease

Vitals Vault peptide care starts with a blood panel and a licensed clinician, and ends with a retest.

## Insulin resistance is an energy problem

Insulin resistance is usually described as a sugar problem. One level deeper, it is an energy problem. Muscle that cannot take up and burn fuel well leaves glucose and insulin high.

Exercise raises your own MOTS-c, and genetic data suggest activity shapes how much it matters. The peptide is best understood as a messenger that amplifies what training already does.[3](https://www.vitalsvault.com/peptides/mots-c#source-lee-2015)[5](https://www.vitalsvault.com/peptides/mots-c#source-reynolds-2021)[6](https://www.vitalsvault.com/peptides/mots-c#source-zempo-2021)

### What else moves the same system

1. 01Keep trainingExercise raises your own MOTS-c, so keep training throughout any course.
2. 02Track HbA1cIt reflects about three months of blood sugar, so that is the earliest sensible retest.
3. 03Check insulin and glucoseFasting insulin and glucose show how the whole system is trending.
4. 04Review your medicinesIf you take diabetes medicine, involve your clinician and check blood sugar.

## Labs to check with MOTS-c

A blood test before you start and a retest after shows what actually changed for you. It is how Vitals Vault approaches every treatment.

1. TestA baseline panel at Quest Diagnostics.
2. DecideA licensed clinician reviews your history and results.
3. RetestThe same markers again, side by side.

## MOTS-c questions

What is MOTS-c used for?

MOTS-c is studied for insulin sensitivity, weight and physical performance with age. People ask about it for metabolic health and exercise capacity. It is not FDA-approved to treat any condition.

What are the benefits of MOTS-c?

In mice, MOTS-c prevented insulin resistance and diet-induced weight gain, and improved physical performance in young, middle-aged and old animals. In people, levels rise with exercise, and a less active natural variant is linked to type 2 diabetes in inactive men.

Is MOTS-c an exercise mimetic?

It is often called one. It activates AMPK, the same energy pathway exercise switches on, and muscle releases more MOTS-c during exercise. It is best thought of as part of how exercise works, rather than a substitute for it.

How does MOTS-c work?

It acts mainly on skeletal muscle. It slows the folate cycle, which switches on AMPK and improves how muscle takes up and uses glucose. Under metabolic stress it also enters the nucleus and helps turn on protective genes.

What are the side effects of MOTS-c?

Human safety data have not been published. Because it affects blood sugar, people on diabetes medicine should be monitored by a clinician.

What is the MOTS-c dosage?

There is no FDA-approved dose, and no human dose has been published: FDA found no clinical studies giving MOTS-c to people in its July 2026 review, and the first placebo-controlled trial has not reported results. Your clinician sets any dose.

MOTS-c vs humanin: what is the difference?

Both are peptides coded in mitochondrial DNA. MOTS-c comes from the 12S rRNA gene and is studied for muscle, exercise and glucose use. Humanin comes from the 16S rRNA gene and is studied mostly in brain and metabolic research, largely in animals and cells.

Is MOTS-c banned in sports?

Yes. The World Anti-Doping Agency’s 2026 Prohibited List names MOTS-c among AMPK activators, a class of metabolic modulators prohibited at all times.

Is MOTS-c FDA approved?

No. MOTS-c is not FDA-approved for any use. Since October 29, 2026, FDA allows 503A compounding pharmacies to prepare it for individual patients with a prescription, under enforcement discretion, while final rulemaking is pending.

Is MOTS-c legal in the US?

A licensed prescriber can now order compounded MOTS-c from a 503A pharmacy for a specific patient. Vials sold online as "research use only" are not made for people and are not covered by FDA’s October 2026 policy.
