## The answer, in short

On October 29, 2026, NCPA reported that FDA granted enforcement discretion so 503A compounding pharmacies can make six peptides: BPC-157, TB-500, epitalon, KPV, MOTS-c and Semax. You need a prescription. None of the six is FDA-approved, no end date was given, and the final rule is still pending.

## What changed in October 2026

FDA granted enforcement discretion for six peptide bulk substances, so 503A compounding pharmacies can prepare them for individual patients. This followed a July 2026 vote by FDA’s Pharmacy Compounding Advisory Committee recommending the six for the 503A bulks list. The committee voted against a seventh peptide, emideltide.

The six peptides covered
| Peptide | Studied for | Usual form |
| --- | --- | --- |
| BPC-157 | Tendon, ligament, muscle and gut repair | Injection or oral |
| TB-500 | Wound, soft-tissue and heart repair | Injection |
| MOTS-c | Insulin sensitivity, weight, exercise capacity | Injection |
| KPV | Gut and skin inflammation | Oral, injection or topical |
| Semax | Stroke recovery, memory and focus | Nasal |
| Epitalon | Telomeres, sleep rhythm, healthy ageing | Injection or nasal |

## What enforcement discretion means for you

- 01

### Pharmacy-made, with a prescription

A licensed prescriber orders the peptide for you, and a 503A pharmacy prepares it under pharmacy standards.[2](https://www.vitalsvault.com/peptides/fda-peptide-compounding-2026#source-fda-503a)

- 02

### Not the same as approval

FDA approval requires trials for a specific use. Enforcement discretion means FDA will not prioritise action against this compounding while the rule is finalised.[1](https://www.vitalsvault.com/peptides/fda-peptide-compounding-2026#source-ncpa)

- 03

### Research vials are still out

Products sold as "research use only" are not made for people and are not covered by the policy.[2](https://www.vitalsvault.com/peptides/fda-peptide-compounding-2026#source-fda-503a)

- 04

### Watch for the final rule

No end date was given. The final rule could make the change permanent or adjust it.[1](https://www.vitalsvault.com/peptides/fda-peptide-compounding-2026#source-ncpa)

## Which peptides are FDA approved?

Many peptide medicines are FDA-approved, including insulin, GLP-1 medicines such as semaglutide and tirzepatide, and teriparatide for osteoporosis. The six peptides in this policy are not approved. They are compounded for individual patients, without FDA review of the finished product.

## Questions

Are peptides FDA approved?

Some are, such as insulin and GLP-1 medicines. BPC-157, TB-500, epitalon, KPV, MOTS-c and Semax are not, but since October 29, 2026, 503A pharmacies can compound them with a prescription.

Is BPC-157 legal now?

A licensed prescriber can now order compounded BPC-157 from a 503A pharmacy. It is still not FDA-approved and is still prohibited in tested sport.

How long will the enforcement discretion last?

No end date was given. Final rulemaking is pending.

Does this apply to 503B outsourcing facilities?

The NCPA report covers 503A pharmacies, which compound for individual patients.

Can I buy these peptides online without a prescription?

"Research use only" products are not made for people and are not covered. Compounded peptides for a person need a prescription.
