## Cerebrolysin, in short

Cerebrolysin is a mixture of hundreds of small peptides and amino acids made by digesting pig brain proteins, given by intravenous infusion in Russia, Eastern Europe, China and elsewhere for stroke and dementia. It is thought to mimic the brain’s growth factors. Many human trials exist, but Cochrane reviews find little effect on death after stroke and weak dementia evidence.

## How does Cerebrolysin work?

Cerebrolysin is a pig-brain mixture described as acting like the brain’s own growth signals.

-

### Acts like growth signals

A review describes it as acting like the brain’s own growth factors, the signals that help nerve cells grow and survive.[6](https://www.vitalsvault.com/peptides/cerebrolysin#source-rejdak-2023)

-

### No known growth signal found

A lab test of one sample found 638 peptides, mostly from structural proteins, and none of the known growth factors it looked for.[9](https://www.vitalsvault.com/peptides/cerebrolysin#source-gevaert-2015)

-

### Proposed brain-repair effects

Proposed effects include new connections, new nerve cells, new blood vessels and less inflammation. Which part does the work is not known.[6](https://www.vitalsvault.com/peptides/cerebrolysin#source-rejdak-2023)

## Cerebrolysin benefits: what it is studied for

All of this evidence comes from people, and it is mixed. The largest trial and the two Cochrane reviews are the most reliable summary, and none shows a clear benefit.

### Stroke

CASTA, a trial of 1,070 stroke patients, found no clear difference on its main outcome. A Cochrane review found little or no difference in deaths.[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)

### Stroke rehabilitation

CARS, a smaller trial with manufacturer staff among its authors, reported better arm and hand function at day 90 and called for a larger trial.[4](https://www.vitalsvault.com/peptides/cerebrolysin#source-muresanu-2016)

3.

### Vascular dementia

A Cochrane review of six trials (597 people) found a small gain in thinking scores. The evidence was very low quality and the gain may be too small to matter.[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)

### Alzheimer’s disease

A meta-analysis of six placebo-controlled trials found better thinking scores than placebo at 4 weeks. At 6 months the gain was no longer statistically significant.[3](https://www.vitalsvault.com/peptides/cerebrolysin#source-gauthier-2015)

## A medicine with no single active ingredient

Take proteins from pig brain, digest them under controlled conditions into a mixture of small peptides and amino acids, and infuse the result over a course of days. That, in outline, is Cerebrolysin. In Russia, Eastern Europe, China and a number of other Asian and post-Soviet countries, doctors have used it widely for acute ischaemic stroke, the kind caused by a blocked artery, and for dementia.

It sits apart from everything else in this guide. Most peptides here are a single, defined chain of amino acids that a chemist can write down. Cerebrolysin is a mixture of many low-molecular-weight peptides and amino acids, closer to a biological extract than to a synthetic drug. There is no one molecule to point to.

It is also unusually well tested. Cerebrolysin has been through multiple randomized trials in stroke, including one with more than a thousand patients, and has two Cochrane reviews of its own. That gives us something rare for a peptide, enough human data to see where the early promise held up and where it did not.

To read that data well, you first need to know what is in the vial, and that turned out to be a harder question than it sounds.[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)

## What is actually in Cerebrolysin?

For a medicine used for decades, its chemical makeup was surprisingly poorly described. In 2015, a drug-quality group at Ghent University in Belgium took a sample of Cerebrolysin bought on the internet and analyzed it with two combined liquid chromatography and mass spectrometry methods, matching what they found against a database of pig proteins.

They identified 638 unique peptides. Most came from a handful of very ordinary brain proteins: tubulin and actin, which form the internal scaffolding of cells, and myelin basic protein, part of the insulating sheath around nerve fibers. In other words, much of the mixture is the broken-down framework of brain tissue.

The team also looked for fragments of the brain’s best-known growth factors, including BDNF, nerve growth factor, glial cell-derived neurotrophic factor and ciliary neurotrophic factor. In this sample, they found none. Whatever activity Cerebrolysin has, the authors concluded, is likely to come from new, previously unknown cryptic peptides, small sequences hidden inside larger proteins.

This was one internet-obtained sample, it cannot speak for every batch, and the authors did not test which peptides do what. But it reframes the product. Cerebrolysin is often described as a cocktail of brain growth factors. This analysis found mostly structural protein fragments and none of the growth-factor pieces it searched for.[9](https://www.vitalsvault.com/peptides/cerebrolysin#source-gevaert-2015)

## How Cerebrolysin works: acting like growth factors

Neurotrophic factors are proteins that keep neurons alive, help them branch and connect, and support repair after injury. A 2023 review in Medicinal Research Reviews describes Cerebrolysin as resembling the activities of several of them, including nerve growth factor, IGF-1 and BDNF, and as changing how much of these factors the brain makes on its own.

The same review links Cerebrolysin to four broad processes after brain injury: neuroplasticity, the rewiring of connections; neurogenesis, the birth of new neurons; angiogenesis, the growth of new blood vessels; and inflammation. These processes may contribute to how much function returns after a stroke or a head injury, alongside the injury itself, the treatment and rehabilitation.

Put this beside the Ghent analysis and you get an honest puzzle. Cerebrolysin appears to behave like growth factors in laboratory work, yet the sample analyzed showed no detectable fragments of the growth factors tested. One way to reconcile the two is that some of its many small peptides nudge the brain into making more of its own factors. That is a hypothesis. Which fragments matter, and how much of each reaches the brain after an infusion, is still undefined.[6](https://www.vitalsvault.com/peptides/cerebrolysin#source-rejdak-2023)[9](https://www.vitalsvault.com/peptides/cerebrolysin#source-gevaert-2015)

## Cerebrolysin for stroke: what the big trials found

The largest test was CASTA, the Cerebrolysin Acute Stroke Treatment in Asia trial, published in Stroke in 2012. It enrolled 1,070 people with acute ischaemic stroke, randomized within 12 hours of symptoms to Cerebrolysin or saline infusions, on top of aspirin, and followed them for 90 days. The main outcome combined three standard measures of disability and neurological function.

That main outcome showed no statistically significant difference. In a post hoc look at people with more severe strokes, the authors saw a trend favoring Cerebrolysin, and lower 90-day deaths in that subgroup, 10.5 percent against 20.2 percent on placebo. Post hoc means the comparison was chosen after the results were in, so the authors themselves said it needed confirming in another trial.

CARS, published in 2016, asked a different question. It paired Cerebrolysin or placebo with a standardized early rehabilitation program and measured arm and hand function at day 90. Cerebrolysin showed a large advantage on that measure and a small-to-medium advantage on global status. Several authors worked for the manufacturer, and the trial was described as exploratory with a relatively small sample.

The 2023 Cochrane review included seven randomized trials and 1,773 people, one of them testing Cortexin rather than Cerebrolysin, and its death analysis pooled six trials with 1,689 people. It concluded that Cerebrolysin or a similar preparation, Cortexin, probably makes little or no difference to death from any cause after acute ischaemic stroke. None of the trials reported the combined outcome of death or dependence at the end of follow-up, the outcome families care about most. The manufacturer supported three of the multicenter trials.[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)[4](https://www.vitalsvault.com/peptides/cerebrolysin#source-muresanu-2016)[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)

## Cerebrolysin for dementia and Alzheimer’s disease

Vascular dementia is the second most common cause of dementia, and it has no established drug treatment. The 2019 Cochrane review found six randomized trials of Cerebrolysin with 597 people, most with mild to moderate disease. No new trials had appeared since the previous version of the review in 2013.

Pooled, Cerebrolysin improved cognition scores and global function. The reviewers rated both findings very low quality, because the trials differed widely and carried a high risk of bias, and every trial with known funding was supported by industry. They concluded that any benefit may be too small to be clinically meaningful and called the supporting evidence base weak.

For Alzheimer’s disease, a 2015 meta-analysis of six placebo-controlled trials in mild to moderate disease reported better cognition at 4 weeks. At 6 months, the cognition difference was no longer statistically significant, although measures of global clinical change still favored Cerebrolysin. The cognition estimate at 6 months was close to the 4-week one but far less certain, so the data cannot say whether the gain held or faded. That uncertainty is worth knowing before anyone commits to a course of infusions.[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)[3](https://www.vitalsvault.com/peptides/cerebrolysin#source-gauthier-2015)

## Is Cerebrolysin safe? Two reviews, one disagreement

Ask whether Cerebrolysin is safe and you will meet two serious analyses that seem to give different answers. The 2023 Cochrane review reported moderate-certainty evidence of more non-fatal serious adverse events with Cerebrolysin, more than twice the rate on placebo. A 2021 safety meta-analysis of 12 randomized stroke trials and 2,202 patients found no significant differences from placebo at all.

Look closer and the two agree more than they disagree. Cochrane also found that Cerebrolysin probably makes little or no difference to the total number of people with serious adverse events, and no clear difference in fatal ones, though that estimate was wide enough to allow benefit or harm. The signal appears only when non-fatal events are separated out, in three trials with 1,335 people. The 2021 analysis, which pooled a larger set of trials, did not find it.

The two reviews asked slightly different questions of different trial sets. Cochrane included only trials that started treatment within 48 hours of stroke and compared it with placebo or no treatment. The 2021 review focused on neurorecovery trials and reported the lowest serious adverse event rate with the most intensive courses, which its authors read as a possible protective effect.

So the honest summary is narrow. Deaths and the overall count of serious problems look similar with and without Cerebrolysin. On non-fatal serious events, Cochrane rated the evidence of an increase as moderate certainty, and the 2021 analysis did not find a significant difference. With several trials at high or unclear risk of bias, that signal deserves weight until a better trial settles it.[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)[5](https://www.vitalsvault.com/peptides/cerebrolysin#source-strilciuc-2021)

## What we actually know in people

Cerebrolysin has one of the larger human records in this guide: a 1,070-person stroke trial, a rehabilitation trial, seven trials in the Cochrane stroke review, six in vascular dementia and six in Alzheimer’s disease. Quantity is not its problem.

Quality and consistency are. The single largest trial was neutral on its main outcome. The most positive results come from smaller, exploratory or industry-supported studies, and the independent Cochrane reviews rate the evidence from very low to moderate certainty, with no clear benefit on the outcomes that matter most.

That does not prove Cerebrolysin is useless for everyone. It does mean the burden of proof has not been met, and that subgroup hints, like severe stroke in CASTA, are hypotheses still waiting for their own trial.[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)[3](https://www.vitalsvault.com/peptides/cerebrolysin#source-gauthier-2015)

## Quality, and who should be careful

Cerebrolysin is approved as a medicine in some countries, yet it is also easily available on the internet worldwide, as the Ghent team noted. A sample bought that way is what they analyzed. For an injectable made from animal tissue, the chain from manufacturer to vein matters a great deal, and buying from an unverified online supplier leaves authenticity, storage and handling uncertain.

The dementia research adds a little reassurance with a large caveat. In the vascular dementia review, the two trials that reported adverse effects in a usable form, with 379 people, showed no difference from placebo, though the reviewers rated that evidence very low quality. The Alzheimer’s meta-analysis also described safety as comparable to placebo. Neither was designed to catch rare or delayed problems.

Some people should be especially cautious and involve their own doctors: anyone who has had a stroke or lives with dementia, people with seizure disorders or kidney disease, anyone with an allergy to pork-derived products, and anyone pregnant or breastfeeding. The reviews cited here do not establish safety in these groups, so the product’s own prescribing information is the place to check. Stroke and dementia care belongs with a neurologist, never with an online supplier.[9](https://www.vitalsvault.com/peptides/cerebrolysin#source-gevaert-2015)[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)

## Labs that target the vascular root of stroke and dementia

Stroke and vascular dementia are, at heart, blood vessel diseases, and the markers that predict them are measurable now. ApoB counts the cholesterol-carrying particles that build plaque in arteries, including the arteries that feed the brain. HbA1c is your average blood sugar over about three months. It does not measure brain vessels directly, but years of high blood sugar are linked to small-vessel damage.

Homocysteine, an amino acid that rises when B vitamins run low, is linked with stroke and memory decline. hs-CRP reflects inflammation anywhere in the body, which drives plaque instability. None of these is a Cerebrolysin test, and none measures how anyone would respond to it. They are general markers of vascular and metabolic health, used where a clinician judges they fit.

This is how Vitals Vault approaches brain health. A blood panel at Quest Diagnostics comes first, a licensed clinician reviews your results and history, and the plan focuses on the risk factors with the strongest evidence. Retesting shows whether they are moving.[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)

## Is Cerebrolysin FDA approved? Where it stands in 2026

Cerebrolysin is not an FDA-approved drug and does not appear in FDA’s approved drug database. It is not on FDA’s compounding safety page, and as a mixture rather than a single peptide it was not among the six peptides covered by FDA’s October 29, 2026 enforcement discretion for 503A pharmacies.

None of the sources cited here establishes a routine US prescribing or compounding route. Products sold online outside regulated channels may not contain what the label says.[10](https://www.vitalsvault.com/peptides/cerebrolysin#source-drugs-fda)[11](https://www.vitalsvault.com/peptides/cerebrolysin#source-fda-bulks)[12](https://www.vitalsvault.com/peptides/cerebrolysin#source-ncpa)

## What this means for you

Cerebrolysin is a pig-brain peptide mixture with a long history, hundreds of identified fragments, no single active ingredient and more human trials than most products here. Those trials, read together, show little effect on death after stroke, weak evidence in dementia and an unsettled question about non-fatal serious side effects.

If you or a parent are facing stroke or dementia, the question worth taking to a neurologist is which treatments have the strongest evidence for this situation, and what is being done about blood pressure, ApoB and blood sugar. Those are the levers that change the odds before the next injury.[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)[9](https://www.vitalsvault.com/peptides/cerebrolysin#source-gevaert-2015)

## What to expect with Cerebrolysin

Where it is used, Cerebrolysin is given as a course of intravenous infusions in a clinic, with outcomes in trials measured at around 90 days for stroke and up to 6 months for dementia.

On timing, the Alzheimer’s meta-analysis found a cognitive gain at 4 weeks that was no longer statistically significant at 6 months. Whether the effect lasts is uncertain, which is worth discussing with a neurologist before considering it anywhere it is available.[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)[4](https://www.vitalsvault.com/peptides/cerebrolysin#source-muresanu-2016)[3](https://www.vitalsvault.com/peptides/cerebrolysin#source-gauthier-2015)

### Cerebrolysin is made of brain growth factors like BDNF and NGF

A mass-spectrometry analysis of one sample found 638 peptides, mostly from structural proteins, and no detectable fragments of BDNF, NGF or the other known growth factors it looked for. It may act like growth factors, but in that sample it did not appear to contain them.[9](https://www.vitalsvault.com/peptides/cerebrolysin#source-gevaert-2015)[6](https://www.vitalsvault.com/peptides/cerebrolysin#source-rejdak-2023)

### Cerebrolysin is proven to improve stroke recovery

The largest trial, with 1,070 patients, was neutral on its main outcome, and the Cochrane review found little or no effect on death. A smaller, manufacturer-linked rehabilitation trial was positive and called for confirmation.[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)[4](https://www.vitalsvault.com/peptides/cerebrolysin#source-muresanu-2016)

### Cerebrolysin has no side effects

Total adverse events looked similar to placebo, but the 2023 Cochrane review found moderate-certainty evidence of more non-fatal serious adverse events. A 2021 meta-analysis did not find a significant difference, so the signal is worth taking seriously.[1](https://www.vitalsvault.com/peptides/cerebrolysin#source-ziganshina-2023)[5](https://www.vitalsvault.com/peptides/cerebrolysin#source-strilciuc-2021)

## How is Cerebrolysin taken?

Where it is used, Cerebrolysin is given as a course of intravenous infusions in a clinic or hospital. It is not a nasal or oral product and has no US prescription form.[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)

## Cerebrolysin dosage

There is no FDA-approved dose, because it is not an FDA-approved drug. In stroke and Alzheimer’s trials it was given as a daily intravenous infusion for 10 to 21 days, or five days a week for four weeks followed by twice-weekly infusions; the doses studied are listed in the table on this page. Where it is used, your physician sets the dose and length of the course.[8](https://www.vitalsvault.com/peptides/cerebrolysin#source-heiss-2012)[4](https://www.vitalsvault.com/peptides/cerebrolysin#source-muresanu-2016)[7](https://www.vitalsvault.com/peptides/cerebrolysin#source-alvarez-2006)

## Typical Cerebrolysin dosage

Cerebrolysin is given as an intravenous infusion, measured by volume. Large stroke trials gave one infusion a day for 10 to 21 days, and Alzheimer’s trials gave infusions five days a week for four weeks, then twice a week. There is no FDA-approved dose, and where it is used a physician sets the dose and course.

These are reference doses from FDA labels, studies in people and clinical practice reports, shown for education. They are not a recommendation or a prescription. Your dose, form and schedule should be set by a licensed clinician who knows your history and lab results.

## Is Cerebrolysin right for you?

A clinician makes the call. These are the questions they will ask first.

### It may suit

- Adults and families who want to understand what the evidence says before considering any brain-health product
- People who want vascular risk factors measured and managed first

### Talk to a clinician first if you are

- Having had a stroke or living with dementia (care belongs with your neurologist)
- A seizure disorder (check the product’s prescribing information)
- Living with kidney disease (check the product’s prescribing information)
- Allergy to pork-derived products
- Pregnant or breastfeeding

Vitals Vault peptide care starts with a blood panel and a licensed clinician, and ends with a retest.

## Stroke and dementia share a vascular root

Stroke and vascular dementia are, at heart, blood vessel diseases, and the markers that predict them can be measured now. A product that tries to protect neurons after an injury works downstream of what damaged the vessels.

Blood pressure, ApoB and blood sugar are the levers to take to a neurologist, because they change the odds before the next injury.[2](https://www.vitalsvault.com/peptides/cerebrolysin#source-cui-2019)

### What else moves the same system

1. 01Control blood pressureWorth raising with your neurologist as one of the levers that change the odds before the next injury.
2. 02Lower ApoBApoB counts the particles that build plaque in arteries, including those that supply the brain.
3. 03Fix blood sugarYears of high blood sugar are linked to vessel disease, so HbA1c is worth tracking.
4. 04Move and sleepExercise and good sleep are everyday habits to review with your clinician.

## Is Cerebrolysin FDA approved?

No. Cerebrolysin is not an FDA-approved drug. It is a pig-brain extract rather than a single peptide, it does not appear on FDA’s compounding safety page, and it is not one of the six peptides covered by FDA’s October 2026 enforcement discretion.[10](https://www.vitalsvault.com/peptides/cerebrolysin#source-drugs-fda)[11](https://www.vitalsvault.com/peptides/cerebrolysin#source-fda-bulks)[12](https://www.vitalsvault.com/peptides/cerebrolysin#source-ncpa)

## Cerebrolysin questions

What is Cerebrolysin used for?

In Russia, Eastern Europe, China and other countries it is used for acute ischaemic stroke and for dementia, given as a course of intravenous infusions. In the US it is not FDA-approved for any use.

What are the benefits of Cerebrolysin?

The largest stroke trial was neutral on its main outcome, and the Cochrane stroke review found little or no effect on death. The vascular dementia review found a small, very low-quality gain in cognition. A meta-analysis in Alzheimer’s disease found a benefit at 4 weeks that was not significant at 6 months.

Is Cerebrolysin a peptide?

It is a mixture of hundreds of small peptides and amino acids from pig brain, not a single peptide. One analysis identified 638 distinct peptides, so it has no single active ingredient.

How does Cerebrolysin work?

It is described as acting like neurotrophic factors such as BDNF and nerve growth factor and as changing the brain’s own production of them. A mass-spectrometry study found no fragments of those growth factors in the mixture, so which ingredients act is unknown.

What are the side effects of Cerebrolysin?

Cochrane found no clear difference in total adverse events after stroke but more non-fatal serious adverse events with Cerebrolysin. Another meta-analysis of 12 trials found no significant difference in serious adverse events.

What is the Cerebrolysin dosage?

There is no FDA-approved dose. In stroke trials it was given as one intravenous infusion a day for 10 to 21 days, and in Alzheimer’s trials five days a week for four weeks, then twice a week. The doses studied are listed in the dosage table on this page. Where it is used, your physician sets the dose and length of the course.

What is the difference between Cerebrolysin and Semax?

Cerebrolysin is an infused pig-brain extract with many ingredients. Semax is a synthetic seven-amino-acid peptide given by nose. Both are used in Russia for stroke, and neither is FDA-approved.

Is Cerebrolysin FDA approved or legal in the US?

No. It is not an FDA-approved drug and is not covered by FDA’s October 2026 compounding policy. Products sold online outside regulated channels may not contain what the label says.
